Inspection history
DOHMH inspection records, newest first, with each violation's official severity and current OPEN/CORRECTED status. We show the government record verbatim and don't grade it.
How to read this record
- Critical — DOHMH's classification for violations that pose a risk to children's health or safety. The center must correct these, and DOHMH follows up.
- General — lower-risk Health Code violations. They still must be corrected.
- OPEN — not yet recorded as corrected in the latest data. CORRECTED — fixed, and the correction is on the record.
- DOHMH inspects licensed centers routinely, after complaints, and to verify corrections — so a violation on record is normal; what matters is severity, whether it is still open, and whether it repeats. Details in our methodology.
Exits and other egress Not provided with required signage at time of inspection.
